Really it varies according to your needs and what's going on for you. Dealing with a more immediate acute problem, such as recent physical (depending on the nature and appropriateness) or emotional trauma might require a number of regular weekly visits to get stabilized.
More long-term chronic issues would respond well with every two or three weeks over a course of time.
As far as being a part of a wellness maintenance plan, once or twice a month is fine. (For me, as someone who is healthy but works regularly on other people, and has a 'normally' stressful life as a mom, every two weeks is ideal.)
I usually say that once you've had bodywork a few times, you begin to develop a sense of what your body needs, and when it's giving you the hint to make that appointment. You'll know when you're due!
